Disability Claim Examples
A 55-year-old male member was disabled as a result of an automobile accident. This accident resulted in head injuries, concussion, headaches, back and neck complaints.
A 52-year-old female member has been disabled due to a torn ligament in her knee as the result of an on-the-job accident. Her activities are very limited in that she cannot climb stairs or walk more than 2 blocks. She requires the use of a knee brace and a cane.
A 50-year-old member has received disability benefits since January 1997. She is disabled as a result of migraine headaches, myalgia, and a jaw disorder. She is unable to work since the jaw problem, which makes speech difficult. The myalgia makes movement painful and the migraine headaches are severe.
